Three officers responded to a call from the man's family that he was mentally ill, that "the effectiveness of the man's medication seemed to be wearing off" and that he was breaking furniture and attacking them with a knife. After removing the family from the home, the officers report that they found the man on his bed with several knives. They report that they commanded him to disarm, that he threatened them, and that he reached for a large knife. Two officers shot him and another tasered him. He died at the scene.
